Guidelines: Nominees must be a resident of Florida (defined as the Florida Fire District) or previously resided in Florida or be associated either through employment or business. Awards to be Presented (CIRCLE ONE) ● Citizen of the Year —This award shall be based on services rendered over a period of time or outstanding services rendered in one calendar year. ● Youth Achievement Award —This award recognizes a youth who has performed outstanding work with school organizations and/or local civic organizations. ● Lifetime Achievement Award —This award is for a person living or deceased and who has contributed a lifetime of service to a local organization and/or the community.
